Summary of the Liu article
The University of Pittsburg Medical Center (UPMC) investigated the rate of electrical failure in Riata leads from
329 patients who underwent fluoroscopic screening from December 2011 to March 2012 at UPMC hospitals. This
publication is the follow-up to the presentation given at HRS 20134

Key takeaways:
-The authors state that there was a
6.6% rate per year of electrical failure for Riata leads with
externalized  conductors
and recommends that patients be considered to undergo lead replacement (the total
number of electrical failures in this study is 5).
-76 out of 329
(23%) patients with Riata leads who underwent screening had cable externalization via
fluoroscopy
-3 of the leads had electrical failure at the time of screening (mean dwell time since implantation was 6.1 ± 0.7
years)
-During a mean 7.9± 2.9 months of follow-up, two additional electrical failures occurred
-The authors only followed patients with externalized conductors and did not provide electrical failure rates on
patients without externalized conductors. Multiple studies 5,6 (including SJM Riata Lead Evaluation Study 7) have
shown there is no correlation between electrical failures and externalized conductors
-Electrical failure was defined as one of the following compared with baseline at the time of screening or last
normal follow-up:
  1.      R waves of <2 mV with ≥50% decrease in R-wave amplitude
  2.      ≥50% increase or decrease in pace/sense or high-voltage lead impedance
  3.      ≥50% increase in pacing threshold
  4.      New electrical oversensing in the near-field or far-field ICD lead channels
